Event Waiver and Acknowledgement of Risk ____ (Event name) I, ____ (Your full name), want to take part in the event named above on ____ (Date) (the “Event”), run by ____ (Organiser) (the “Organiser”). Section 1: I understand the risks 1.1 The Event can involve physical effort, travel, equipment and other people, and so carries a risk of injury, illness, and loss of or damage to property, even when everyone is careful. In some cases the injury can be serious. 1.2 I am choosing to take part. I am in good enough health and fit enough to do so, and I have not been told by a doctor not to take part. 1.3 I will tell the Organiser before the Event if my health changes in a way that could affect my safety or anyone else’s. Section 2: I will follow the rules 2.1 I will follow the safety rules and instructions that the Organiser and its staff give, and use any protective equipment I am asked to use. I will not take part if I am affected by alcohol or drugs. 2.2 The Organiser may refuse me entry to, or ask me to leave, the Event if I do not follow the rules or if my behaviour is a danger to others, and I will not be entitled to a refund. Section 3: What I accept 3.1 I accept the risks that are a normal and expected part of the Event. I agree that I will not bring a claim against the Organiser, its staff or volunteers for injury or loss that results from those risks. 3.2 This does not apply to anything caused by the Organiser’s own negligence or deliberate wrongdoing, and it does not limit any right that the law does not allow me to give up. Nothing here removes the Organiser’s duty to run the Event with reasonable care. Section 4: Equipment, property and safety 4.1 I will use any equipment provided by the Organiser only as it was meant to be used and following the instructions, and I will check my own equipment is in good condition before I use it. I will tell the Organiser at once if I find something that is damaged or unsafe. 4.2 I am responsible for my own belongings. The Organiser is not responsible for loss of or damage to them, except where it was caused by the Organiser’s negligence. I will pay for damage that I cause to the Organiser’s equipment or the venue by misuse. Section 5: Medical help and insurance 5.1 I understand that first aid or medical help at the Event may be limited. I am responsible for having whatever medical or accident insurance I need, and for my own travel to and from the Event. 5.2 I will take any medication I need with me and will tell a member of staff where it is. If I have a condition that could affect others, such as a severe allergy, I will tell the Organiser before the Event. Section 6: Changes to the Event 6.1 The Organiser may change, postpone or cancel the Event because of weather, safety or other reasons outside its control. If it cancels, it will tell me promptly, and any refund will be as described when I registered. Section 7: Governing law 7.1 This document, and any dispute arising out of it, is governed by the laws of the State of ____ (Governing law (state)), without regard to its rules about conflicts of laws. ____ (I have read this waiver and I understand it. Questions

Who fills in which part?

Participant (seat 1) fills in and signs. In the preview, every box is drawn where it will sit on the page.

Can I change the wording?

Yes. Once it is in your workspace it is your own template: open it, edit the text or the fields, and save. The original starter is never changed.

Is this legal advice?

No. It is a starting point written in plain words. Change anything that does not fit your situation before you send it. It makes no promise that it is enough for any place or any purpose.
